Hospice is about life, not death
Serenity Palliative Care and Hospice realizes that not everyone fits into the “standard” Hospice patient mold.  Therefore we have started Four different programs in addition to our outstanding Hospice services they are:
           
Community Wellness Program
Serenity is committed to improving the life of our patients and the community.  For this reason we have developed and are implementing our Community Wellness Program.  We work with Retirement Facilities, Assisted living homes, and Senior Centers to perform monthly and health screenings for those that may not have consistent care.  We also work with doctors in the community to give them updates on patients that may not have seen recently.  We also provide a wellness card that tracks vital signs and other critical information that can then be taken to your next Doctor visit.  Our community wellness team consists of an RN, LPN, Licensed Clinical Social Worker, and a Medical Assistant or CNA. 

Education
We have dedicated ourselves to the education of the community and to our colleagues that may provide end-of-life or palliative care.  We primarily do this in Nursing homes and assisted living centers, by providing workshops and in-services.  We are also developing relationships with nursing schools and CNA programs to conduct lectures on end-of-life care and palliation. 

Because, less than 50% of the people eligible for hospice actually receive this specialized care, we feel we can improve end-of-life care by educating those that care for the other 50%.  This is our mission and our goal; to improve end-of-life care for all Oregonians, not just those on our service
