As a leading voice in Oregon hospice care, Serenity Palliative Care and Hospice realizes that not everyone fits into the standard description of a hospice patient. Therefore, in addition to our outstanding hospice care services, we have implemented several different programs that inform and serve our community.
Community Wellness ProgramAt Serenity Hospice we are committed to improving the life of our patients and our communities here in Oregon. For this reason we have gone beyond hospice care and developed our Community Wellness Program. Working with retirement facilities, assisted living homes and senior centers, we perform monthly health screenings for those who may not have consistent care. We also work with doctors in the community, giving them updates on patients that they may not have seen recently. We provide wellness cards that track vital signs and other critical information. These cards are useful at the next doctor visit. Our Community Wellness Team consists of an RN, LPN, Licensed Clinical Social Worker and a Medical Assistant or CNA.
EducationEveryone at Serenity Hospice is dedicated to educating Oregon, our communities and our colleagues on end-of-life hospice or palliative care. We primarily provide workshops and in-service training at nursing homes and assisted living centers. In addition, we’re developing relationships with nursing schools and CNA programs where we conduct lectures on end-of-life care and comfort care.
Since less than 50% of people eligible for hospice care actually receive this valuable, specialized care, we feel we can improve overall end-of-life care by educating caregivers serving the other 50% on how to get hospice care for their patients. This is the Serenity Palliative Care and Hospice mission and our goal; to improve end-of-life care for all Oregonians, not just those we serve.
